Important Information:

  • This service is provided to patients free of charge.
  • Please supply a valid email address as this will be the identifier that is used to access your account.
  • For security purposes, we need to confirm that you are the owner of the email address that you supply. After clicking the Submit button, an email will be sent to you with a link that will enable you to complete the account activation process.
  • Please choose a secure password. Your password must contain at least 8 characters with no spaces and at least 1 number. We recommend that you change your password frequently.
  • To create a new Provider account, you will need to enter a valid Provider Code. If you are an employee of a BHRT Provider, ask them for the Provider Code. Your account will be created, but you won't be able to do Sign-In until your employer approves your request. If you are a brand new Provider, please contact our sales staff to obtain a Provider Code.
